IMPORTANT - Parents/Guardians
If you plan to have your child receive the H1N1 vaccine, you must return the RSVP form (elementary) or the Medical Screening Questionnaire (JMS & LHS) form that is included in the building information below.  Forms are due to your school office by October 28th. 

Siblings not enrolled in Lakeview Public Schools cannot be vaccinated at the school located vaccination clinics.  Please check the Macomb County Health Department website or call the Flu Hotline at (586) 466-7923.  There are two nasal spray H1N1 flu immunization clinics on October 28 and 29 at Freedom Hill in Sterling Heights for healthy people aged 2 to 49 years of age. See the county website for further details as they become available.
